# Diagnoses

Manage the diagnoses available in SOAP assessment for your clinic — adding clinic-specific entries, and why the default ones cannot be edited.

Diagnoses are the reusable medical definitions your team selects during SOAP assessment. They differ from symptoms: symptoms describe what is observed or reported, while a diagnosis is the clinician's assessment.

## Where to Find This Section

1. Open **Clinic Admin**.
2. Go to **Definitions**.
3. Open **Diagnoses**.

## Default and Clinic-Specific Diagnoses

The list holds two kinds of entry, and you can tell them apart at a glance: system-provided entries carry a **Default** badge.

Default diagnoses can be used in assessments but not changed, and their rows have no action menu. Only the diagnoses your clinic created can be edited or deleted.

## How to Add a Diagnosis

1. Open **Definitions → Diagnoses**.
2. Click **Add Diagnosis**.
3. Fill in the form and save.

## Form Fields

### Name

The diagnosis name your team will pick from during SOAP assessment. Keeping the naming style consistent helps staff find the right option quickly.

### Detail

Optional. Use it to clarify how a diagnosis should be used inside your clinic.

## Editing and Deleting

Use the action menu at the right of a clinic-specific row.

> **Caution**
> Deleting is permanent, and the confirmation says so. If a diagnosis is simply no longer in use, consider leaving it in place; historical visit records are easier to read when the names they were written with still exist.

> If your clinic uses SOAP-based visit records, keep diagnosis names stable where you can, so historical records stay easy to read and filter.
