Diagnoses are the reusable medical definitions your team selects during SOAP assessment. They differ from symptoms: symptoms describe what is observed or reported, while a diagnosis is the clinician’s assessment.
Where to Find This Section
Section titled “Where to Find This Section”- Open Clinic Admin.
- Go to Definitions.
- Open Diagnoses.
Default and Clinic-Specific Diagnoses
Section titled “Default and Clinic-Specific Diagnoses”The list holds two kinds of entry, and you can tell them apart at a glance: system-provided entries carry a Default badge.
Default diagnoses can be used in assessments but not changed, and their rows have no action menu. Only the diagnoses your clinic created can be edited or deleted.
How to Add a Diagnosis
Section titled “How to Add a Diagnosis”- Open Definitions → Diagnoses.
- Click Add Diagnosis.
- Fill in the form and save.
Form Fields
Section titled “Form Fields”The diagnosis name your team will pick from during SOAP assessment. Keeping the naming style consistent helps staff find the right option quickly.
Detail
Section titled “Detail”Optional. Use it to clarify how a diagnosis should be used inside your clinic.
Editing and Deleting
Section titled “Editing and Deleting”Use the action menu at the right of a clinic-specific row.
If your clinic uses SOAP-based visit records, keep diagnosis names stable where you can, so historical records stay easy to read and filter.
